The online questionnaire consists of six documents with electronic forms support for data entry. Each part can be downloaded individually. In addition, the user has the option to download the questionnaire integrating the six documents as a whole.
After filling out a specific form, please print a copy for your reference, copy/notify your coordinating authority, and submit the form via e-mail by pressing the "Submit" button at the top of each form. Alternatively, a paper copy of the questionnaire can be submitted via regular mail (through your coordinating authority).
The free Adobe Reader is needed to open/fill out the above mentioned documents and use them as electronic forms that can be printed and/or submitted via e-mail. Download Adobe Reader via the below icon, if it is not already installed on your computer.
To save an additional electronic copy of the completed questionnaire, Adobe Acrobat Professional software is required. This would facilitate the passing on of the partially completed questionnaires in the electronic format but it is not required to respond to the survey. Alternatively, the questionnaire documents in MS Word format available here could be used for this purpose.
